To update annual forms for returning students:

Each spring, a parent/guardian will need to complete the annual student information forms for the upcoming school year. These forms include emergency contacts, consent, medical information, transportation requests, and other necessary information we require annually for each student.

Annual forms are required for all returning students.
Changes to parent/guardian email addresses and phone numbers can be updated throughout the year if there is a change to contact information for BrightArrow communications, the district's email and text platform.

Change of Address

You must attach a copy of this change of address form.

You must also attach one of the following documents:

A purchase agreement,
A lease agreement, or
A building contract.

If your new address changes your assigned school, please visit:
4747 N. Holland Sylvania Road, Monday–Friday, 7:30 a.m. – 4:00 p.m., to obtain a K–8 Redistricting Form.

If you are residing with a current district resident, please provide:

The Change of Address Form with Section C completed & notarized,
The resident’s lease or purchase agreement, and
A current utility bill in the resident’s name.

Within 30 days, you must provide one of the following documents in your name at that address:

A current utility bill,
A W-2,
A pay stub,
A tax document, or
A government-issued document.
