We are happy to announce that Sylvania Schools is partnering with Toledo Lucas County Health Department to provide a vaccination clinic for our students who are 12-15 years of age along with their vaccine-eligible family members

Second Dose
TLCHD has confirmed that it will be the Pfizer vaccine. You will make the second dose appointment on-site at the time of the first dose for Wednesday, June 9. Please see the Emergency Use Authorization Fact Sheet for the Pfizer vaccine (link).
 
Students who are 12-17 
Students who are under 18 years old will need to have a consent form signed by a parent/guardian. A parent/guardian can accompany the minor to the appointment, but it is not necessary. If a parent/guardian will not be with the student at the appointment, then the student must bring the consent form signed by the parent. Consent form (link)

In addition to the vaccination times available through this link, the health department will also offer vaccine appointment times to the public at Southview on the same date through the Lucas County Health Department’s website.

The COVID vaccine is coming! Some of our Sylvania Schools teachers and support staff put together this video to encourage COVID-19 vaccination. We're looking forward to achieving herd immunity in Sylvania!
