Arbor Hills uses Final Forms to handle all paperwork for athletics. All parents and student athletes will be required to create an account and update information as needed. Students/parents must complete all forms on Final Forms before they can tryout or practice and must have a physical on file. 

 

As a reminder, students will use their SYLVANIA SCHOOLS email to create their accounts. Also, make sure to choose the sport that you are playing that season.
