Final Forms and Physicals

Arbor Hills uses Final Forms to handle all paperwork for athletics. All parents and student athletes are required to create an account and update information as needed. Students/parents must complete all forms on Final Forms before they can tryout or practice and must have a physical on file.

As a reminder, students will use their SYLVANIA SCHOOLS email to create their accounts. Also, make sure to choose the sport that you are playing that season.
